Lander Medical Clinic and Western Family Care together are a locally owned, multispecialty group practice.
Our mission is to provide quality, evidence based healthcare to all individuals in our community with dignity, compassion, and respect. We pride ourselves on being Fremont County’s medical home.
Our clinicians are physicians, physician assistants, and nurse practitioners who practice Behavioral Health, Family Medicine, Gynecology, Internal Medicine, Pediatrics, Dietetics, Aesthetics and Same-Day Care in a caring and trusted environment.

Lander Medical Clinic and Western Family Care are proud to have been integral to Fremont County and surrounding area communities for over 70 years. During that time the healthcare industry has seen many changes, but we continue to adapt and strive to be your complete medical home. Our goal is to help all of our patients and family members obtain high-quality health care services at an affordable cost.
We would like to highlight our Patient Portal – a feature that allows our patients better access to their personal health information and provides a state-of-the-art venue for communication and education on health care matters. By providing the tools to empower our patients to take a more active role in managing their health, we can all contribute to reducing the skyrocketing costs of health care. The Patient Portal allows patients to communicate with their medical provider more efficiently. It enables patients to pose questions via email and allows providers to send secure messages to the patient regarding test results and respond to patient inquiries. Live operators will also be available after hours if you need to cancel or reschedule your appointment.
We are also proud to be recognized by the National Center for Quality Assurance as a Patient Centered Medical Home. We feel that as a PCMH we have increased access to care for our communities and provide a better experience for our patients. More specifically, The Joint Principles of the PCMH state that a successful patient-centered medical home has five main parts:
- Comprehensive Care: This is provided by looking at the patient as a whole person, not just focusing on one problem. It also means having a team to help with mental and physical health.
- Patient-Centered: This is provided by building good relationships with patients and their families. Understanding their background, beliefs, and what they need from healthcare is essential.
- Coordinated Care: This is delivered by ensuring everyone involved in a patient’s care knows what’s happening. This might involve sharing health information between different places where the patient gets care, like hospitals or clinics.
- Accessible Services: These are distributed by having flexible hours, offering different ways to get care after normal clinic hours, and using technology like video calls or apps to connect with patients.
- Quality and Safety: This is created using the best methods and tools to ensure patients get safe, effective care that meets their needs.
